How Long Does Seroquel Stay in Your System? The Settled Numbers and the Contested Ones
Quetiapine's mean terminal half-life is about 6 hours for immediate-release SEROQUEL and about 7 hours for SEROQUEL XR, according to the FDA-approved prescribing information published on DailyMed, so the plasma concentration from a final dose falls to a negligible fraction over roughly 30 to 35 hours. Its active metabolite, norquetiapine, has a mean terminal half-life of about 12 hours. Those numbers describe how fast the molecule leaves your blood, and nothing else. They do not tell you when sedation stops slowing your reaction time, whether it is safe to stop taking it, how another medicine has changed your levels, or what a laboratory would report about you. Each of those has a separate answer, and each answer is longer.
The settled part: what the SEROQUEL label reports
Six hours is the figure every other claim is built on. The SEROQUEL prescribing information on DailyMed (set ID 0584dda8-bc3c-48fe-1a90-79608f78e8a0, opened 5 September 2026) states that elimination "is mainly via hepatic metabolism with a mean terminal half-life of about 6 hours within the proposed clinical dose range." Multiply by five, the convention for near-complete elimination, and you get the 30 hours quoted in almost every summary. The arithmetic is sound.
Three further label figures rarely travel alongside it. Steady-state concentrations are expected within two days of dosing, so your blood level stops climbing roughly 48 hours after a dose is set. Immediate-release quetiapine reaches peak plasma concentration at 1.5 hours. And less than 1% of a dose leaves the body as unchanged drug; about 73% is recovered in urine and 20% in feces, nearly all of it as metabolites.
One more variable sits in the same section. Oral clearance was 40% lower in patients aged 65 and over than in younger patients. A mean half-life is a population average, not your clearance.
"In your system" is three questions filed under one code
I spent more than a decade on a Cape Town beauty returns desk, coding what customers said a product had done to them. The codebook kept tightness, stinging and peeling separate on purpose, because they route to different queues and drive different product decisions. Early on I merged tightness with burning, reasoning that both meant irritation. A week of cases went to the wrong queue. We reopened and rerouted them, and that week's pattern report was worthless, because the counts had been pooled before anyone read them. The number was still a number. It answered a question nobody had asked.
"In your system" is a merged code of that kind. It pools three questions: is the molecule still measurable, is it still acting on me, and is my body still adapted to it. The half-life answers the first. The second and third are not pharmacokinetic questions, and no multiple of 6 hours will reach them.
Immediate-release and extended-release: the comparison the overviews skip
Almost every version of this answer gives one clearance figure for a product that ships in two shapes. Formulation changes the numbers.

Total exposure matches. The XR label states that once-daily SEROQUEL XR at steady state has comparable bioavailability to the same total daily dose of immediate-release SEROQUEL given twice daily. What differs is the shape of the curve. A bedtime XR tablet reaches its peak concentration around six hours later, near waking. The food rule carries a number too. A high-fat meal of roughly 800 to 1,000 calories raised XR peak concentration by 44% to 52% for the 50 mg and 300 mg tablets. Norquetiapine's approximately 12-hour half-life applies to both, since it is the same metabolite.
Sedation runs on a different clock than clearance
The label puts somnolence at 18% (89 of 510) of patients in schizophrenia trials against 11% (22 of 206) on placebo, and 57% (398 of 698) against 15% (51 of 347) in bipolar depression trials, "especially during the 3-5 day period of initial dose-titration." It advises caution with driving until you are reasonably certain the drug does not affect you adversely, and notes somnolence may lead to falls.
Measured impairment outlasts the peak. In a randomized, double-blind, placebo-controlled crossover trial published in Annals of the American Thoracic Society in 2026, Fauska and colleagues at Flinders University gave 15 people with obstructive sleep apnea a single 50 mg bedtime dose. The next morning, median reaction time on a psychomotor vigilance task was 382 ms against 336 ms on placebo, and driving-simulator steering deviation was 96 cm against 72 cm. The same paper notes 10.7 million US quetiapine prescriptions dispensed in 2023.
Until roughly 2020 I answered every "how long does it stay in me" question by handing over the half-life multiplication and stopping there. I no longer do. The arithmetic told people when the concentration would be small. It never told them when they could drive.
Why thirty hours is no permission to stop
Here is the strongest case against this article. The half-life arithmetic in the AI overviews is correct. Six hours times five is thirty, the sources repeating it are not lying, and for someone who took two doses and stopped, nothing about clearance is complicated. All of that is true, and all of it is silent on the person taking the drug, which is where the risk lives.
The SEROQUEL label reports that in short-term placebo-controlled monotherapy trials with SEROQUEL XR including a discontinuation phase, one or more discontinuation symptoms after abrupt cessation occurred in 12.1% of patients (241 of 1,993) against 6.7% on placebo (71 of 1,065). No individual reaction exceeded 5.3%, and symptoms usually resolved within a week. "Gradual withdrawal is advised."
How gradual is contested. The antipsychotic deprescribing algorithm by Bjerre and colleagues, published in Canadian Family Physician in 2018, recommends 25% to 50% dose reductions every 1 to 2 weeks with monitoring at the same interval, and applies to people treated 3 months or longer for dementia-related symptoms or for insomnia. That guideline explicitly excludes schizophrenia, schizoaffective disorder and bipolar disorder, directing clinicians to continue treatment or consult a psychiatrist. Horowitz, Taylor and colleagues, writing in Schizophrenia Bulletin in 2021, argue for something far slower: reductions of about a quarter of the most recent dose at intervals of 3 to 6 months, with final steps as small as one fortieth of a therapeutic dose.
Tablet strength settles none of this. SEROQUEL comes in 25, 50, 100, 200, 300 and 400 mg tablets, and a 25 mg tablet may be a starting dose, a hepatic-impairment dose, or the last rung of a long taper.
The liver does this work, and kidney impairment is not the same problem
Quetiapine is extensively metabolized by the liver, and the label's own comparison makes the asymmetry plain. Patients with hepatic impairment had 30% lower mean oral clearance, and in two of eight of them peak concentration and total exposure were three times higher than typically seen in healthy subjects; the labeled starting dose in that population is 25 mg/day, raised in 25 to 50 mg/day increments. Patients with severe renal impairment had 25% lower mean oral clearance, yet their plasma concentrations fell within the range seen in normal subjects, and the label concludes that dosage adjustment is therefore not needed.
Interactions move the number further than either. Ketoconazole 200 mg daily for four days cut quetiapine oral clearance by 84% and raised exposure 6.2-fold, which is why the label instructs reducing the dose to one sixth of the original with a potent CYP3A4 inhibitor. Phenytoin pushed clearance the other way, a 5-fold increase, and the label allows raising the dose up to 5-fold with chronic potent inducers, then returning to the original level within 7 to 14 days of stopping it. Grapefruit, antifungals and certain antivirals sit on the same enzyme.
What a drug test can say about quetiapine, and what it cannot
A standard workplace screen is not looking for it. The SAMHSA Mandatory Guidelines for Federal Workplace Drug Testing Programs authorize testing for marijuana, cocaine, amphetamines, phencyclidine, specified opioids, 6-acetylmorphine and fentanyl. Quetiapine appears nowhere on that panel, and neither do antipsychotics as a class.
Detection windows therefore depend on a test somebody deliberately ordered. Under 1% of a dose leaves unchanged, so a urine assay targeting the parent drug chases a small fraction, while one targeting metabolites measures something else on a different timeline. Any window quoted without an analyte, a method and a cutoff is a number with nothing behind it.
Quetiapine also shows up where it should not. Sloan and colleagues reported a false-positive tricyclic antidepressant screen in American Journal of Psychiatry in 2000; Widschwendter and colleagues documented cross-reactivity with urine methadone immunoassays in the same journal in 2007, and Fischer and colleagues confirmed it in Journal of Clinical Psychiatry in 2010. Brahm and colleagues reviewed 25 such reports in American Journal of Health-System Pharmacy and recommend confirmation by a second analytical method. If a screen surprises you, ask for confirmatory testing before anyone acts on it.
What to ask before you change anything
I have never worked a pharmacy counter and cannot tell you what your taper should be or how your liver is handling this drug. What I can vouch for is reading complaint language for a living. I can tell when a claim has no method behind it, and "stays in your system for X days" almost never has one.
Bring these to the prescriber or pharmacist: which formulation and strength you take, what it was prescribed for, how many days you have taken it continuously, whether any of your other medicines touch CYP3A4, and, if a test is coming, which analyte and cutoff the laboratory uses. Those five answers decide your case. The half-life does not.
